How to add multimedia to forms using the SurveyHeart web application?

Step 1:

First, you must fill in the mandatory Name field for your online survey. You can skip the Description field below it if you want, as it is optional. Survey creators need to pay enough attention to give a suitable title as this will allow the survey takers/survey respondents to understand what the survey is about just by looking at it.

 

Step 2:

In this step, you'll create your first online survey question by clicking the "Add Question" icon in the middle right below the Description field in the Form Builder dashboard. When you're done with that, you'll see a drop-down list with all the available question types.

 

Step 3:

After selecting the appropriate question type for your question, you must enter the question. Once done, you will need to click once on the attachment icon in the right corner of the question box.

 

Step 4:

When you click on the Attachment icon, a drop-down menu will appear with three (3) options for multimedia: Image, Video, and Web. Choose what you think is appropriate for your question; in the case of an image, you will be prompted to upload it from your device, and in the case of a video or web page, you will be prompted to paste the appropriate link to the video/web page directly below the Question box in the question box.

 

After embedding your media, we recommend that you do a preview to test how the image looks or if the video/web link works from the end users' screens, as appropriate.

 

Step 5:

Repeat step 2, step 3, and step 4 for each additional question in your survey if you want to add image, video, or web multimedia options to them.

 

Step 6:

Finally, when you're done with your survey design, you need to hit the Submit button on the left sidebar of the Form Builder dashboard to submit your online survey for publication.
